Just before 2pm on Thursday 2nd October 2014 emergency services were called to an accident at Lochaven Produce, located at Mount Street, Burnie.

An 18 year old male operating a forklift died at the scene as a result of the forklift rolling on him. Ambulance Paramedics were unable to revive the male.

Investigations are continuing and Police are preparing a report for the coroner.

Workplace Standards also attended the scene and are assisting in the investigation.
